Join our team as an Advanced Practice Clinician (APC) - Nurse Practitioner
Location: Portland, OR.
Worksite: Senior Living Facilities.
Job Type: Contract 4-7 months depending on need.
Institutional Special Needs Plan, a type of Medicare Advantage plan for people who need the level of care provided in a long-term care facility for at least 90 days. ISNPs are designed for people who live in or receive nursing care at home, and include benefits tailored to their medical, social, and emotional needs. ISNPs combine the benefits of Original Medicare (Parts A and B) with prescription drug coverage (Part D).
Enjoy work-life balance with our flexible Monday to Friday schedule, 8am-5pm, and **NO** on-call, weekends, or holidays required.
Key Responsibilities:
- Conducting comprehensive assessments
- Managing acute and chronic medical and behavioral conditions
- Collaborating with the care team and ensuring accurate documentation
- Providing counseling, education, and advanced illness planning
Preferred Qualifications:
- 1+ year of clinical experience, preferably in a long-term care setting
- Proficiency in geriatrics, chronic illness, and end-of-life discussions
- Strong computer skills for medical documentation
