Join our team of compassionate and competent professional as a Care Coordinator in the BHCP program at Behavioral Health Network!

The Care Coordinator for the BHCP Program are the individual's primary contact for questions, team decisions, and services. You and your team will empower and promote changes needed to facilitate positive health outcomes with challenging and diverse health populations. You will expand your knowledge base by completing mandatory and interest-guided optional trainings and your working knowledge of community resources

Behavioral Health Community Partners (BHCP):

The Behavioral Health Community Partners (BHCP) team at BHN holds each individual at the core of what it does. Through the coordination and integration of care, BHCP assists adults ages 21-64 with medical, behavioral health, or substance abuse concerns in navigating the health care system and improving their health outcomes.

Qualifications/Requirements

• 1-2 years of work experience in a related mental health field.
• Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management.
• An active Driver's License and access to reliable transportation (required)
• Familiarity with Internet, email systems, and electronic health records (EHRs).
• English-Spanish speaking (preferred)
