How you can make a bigger difference:

The CCBHC is comprised of several community based programs providing direct care to over 3,200 clients annually, both on site and in the community. The Bridges to Care Social Worker reengages clients who have stopped attending treatment and are deemed a risk to harm self or others or an elevated risk for hospitalization. Working primarily in the field, the social worker provides in-home crisis stabilization/treatment services/supports and 24/7 emergency crisis response.

KEY ESSENTIAL FUNCTIONS:
  • Comprehensive assessment of client within 30 days of entry into program.
  • Work closely with team to visit each client a minimum of 6 times monthly.
  • Integration of medical, specialized and behavioral health services in addition to social support and/or educational support services
  • Periodic reassessment of a client's medical and behavioral health needs and update of , adherence to recommended treatments, and need for care coordination
  • Attend scheduled team meetings and one-on-one supervisions
  • Attend mandatory ACT trainings and Jewish Board MKSEI 13 Module course training.
  • When needed/requested, lead team in-service training on topics that will improve service to clients.
  • Collaborative development of an Individualized Care Plan (ICP) with the client, the client's family and/or caregivers in addition to other service providers, and review and revise ICP per regulatory standards or when there are significant changes in functioning or clinical status.
  • Provide care management services
  • Track all specialty medical, behavioral and support service referrals made for patient using Health Information Technology (HIT) provided.
  • Assure that client has access to, engages in and retains needed services as defined in the ICP.
  • Provide outreach services to clients to increase engagement in services identified on individualized care plan
  • Complete all required documentation
  • Provide after-hours emergency coverage via cell phone on rotating basis.
COMPUTER SKILLS REQUIRED (List the computer skills needed):
  • Proficient with electronic health record; Microsoft Office (Word, Powerpoint, Excel, Outlook); comfortable and proficient using audio video platforms (i.e., Zoom).
EDUCATIONAL / TRAINING REQUIRED:

Master's level degree in human services field with appropriate NYS licensure (e.g. LMSW, LMHC, LCSW, MFT, Creative Arts Therapist, Master's or Doctorate in Psychology), or a CASAC or Rehabilitation Counselor.

VISUAL AND MANUAL DEXIERITY:
  • Able to read data and documents, including spreadsheets, reports and Electronic Health Records in printed form and on computer screens.
  • Able to input data into the Electronic Health Record.
  • Limited applications of manual dexterity and hand-eye coordination.
WORK ENVIRONMENT / PHYSICAL EFFORT

Physical Demands:
  • Regularly required to speak clearly and hear the spoken word well.
  • Regularly required to physically operate routine office equipment such as telephones, computers, etc.
  • Regularly required to utilize near vision ability and to read data and documents, including spreadsheets, reports and Electronic Health Records in printed form and on computer screens.
  • Regularly required to travel to various locations for field work in the community.
Work Environment:
  • Noise level is consistent with levels usually present in an office, education, rehabilitation or health related environment when in the office.
  • Hazards present are consistent with those common to an office, education, rehabilitation or health related environment when in the office.
