Salary: $43.00 USD Hourly - $50.00 USD Hourly
Description: Judge Healthcare is currently seeking an RN Case Manager in Orange, CA!

Contract: 3+ months

Hours: Monday-Friday 8a-5p

Location: Onsite in Orange, CA

About the Role: Join our team as a Medical Case Manager, where you'll play a crucial role in providing ongoing case management services for our members. This advanced specialty collaborative practice focuses on facilitating communication and coordination among all participants of the healthcare team to ensure quality and cost-effective outcomes.

Key Responsibilities:

• Care Management: Conduct comprehensive health needs assessments, including physical, functional, social, and psychological evaluations.
• Post-Discharge Assessments: Identify and address post-hospital or emergency department discharge needs.
• Care Plan Development: Create and implement personalized care plans with SMART goals, continuously updating them to reflect changing needs.
• Follow-Up and Monitoring: Schedule follow-ups, assess progress, and identify barriers to achieving goals.
• Outreach: Provide regular outreach to assigned members and evaluate the quality of services provided.

Minimum Qualifications:

• Associate degree in nursing (ADN) or related field.
• 3 years of clinical or managed care experience.
• Equivalent combinations of education and experience may be considered.

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN) or related field.
• 2 years of pediatric experience.
• Certified Case Manager (CCM) certification.
• Bilingual in English and one of the following languages preferred; Arabic, Chinese, Farsi, Korean, Spanish, Vietnamese.

Required Licensure/Certifications:

• Current, unrestricted Registered Nurse (RN) license to practice in California.
