Summary:

The Clinical Intake Coordinator is responsible for the referral process management for skilled and long-term nursing referrals. This position is responsible for optimizing admission capability to the nursing centers, using nursing assessment skills to complete preadmission screens and eligibility verification. The coordinator must possess excellent critical thinking, problem solving, organizational skill and problem solving. The ability to work quickly in a fast-paced environment is required. This position supports regional marketing efforts and promotes the company continuum of care.

Shift: M-F (Hours: 8:00 am - 4:30 pm)

  • Reviews all incoming referrals for admission viability to nursing centers based on admission criteria and bed availability.
  • Utilizing nursing assessment skills, completes a thorough preadmission screening of residents referred to nursing centers to assure a safe transition to the appropriate level of care. This includes the knowledge to determine medical stability, medication outliers, DHS and OBRA compliance, insurance benefits available, foresight of skilled or long term care potential and appropriate interventions related to resident safety issues.
  • Maintains open and proactive communication with all customers. Addresses internal and external issues related to the admission process with appropriate parties, including referral sources, nursing center administration, and external vendors to overcome barriers. Proactively communicates potential admissions during nursing center daily census meetings.
  • Identifies and follows up on opportunities to develop new and existing referral relationships, profitability, efficiency and market share
  • Gathers accurate payor information and verifies insurance coverage and eligibility.
  • Demonstrates appropriate knowledge and competence of designated skills and processes identified for this position, such as software used for CRM, pharmacy, electronic medical record and hospitals EMR, approval processes, internal notifications and tracking.

Education:

Graduated from an accredited School of Nursing and must possess either a current Practical Nurse License (LPN) or a current Registered Nurse license (RN), in good standing, from the Commonwealth of Pennsylvania. Associates or Bachelor's degree in related field preferred.

Experience & Knowledge

At least one year of experience in a medical/surgical unit of an acute care facility. Demonstrated ability to communicate effectively orally and in writing. Must have the ability to use a variety of computer software applications. Working knowledge of computers, Microsoft Word and Excel required.
