Summary: Develops, coordinates, and administers organizational programs for social work, value based care, case management, and utilization review; ensures compliance with The Joint Commission (TJC), New York State Department of Health (NYSDOH), Centers for Medicare and Medicaid Services (CMS) and other regulatory agencies as they apply to the organization; collaborates in the development of value based care programs, including development of necessary policies. Ensures the delivery of high quality and efficient healthcare through administrative and clinical leadership. The salary range for this position is $137,457 - $173,664 annually which includes a comprehensive benefits package.

Qualifications:Required Education and Experience

License Requirement
License to practice as a Registered Nurse in the State of New York; or
New York State Certification as a Licensed Clinical Social Worker (LCSW)

Education and Experience
1. Master's degree in nursing, social work, or a related field and the equivalent of five (5) years of full-time experience in case management/transition of care or a related field, including two (2) years experience in a supervisory, administrative, consultative, managerial or executive capacity; or
2. Bachelor's degree in nursing, social work or a related field and the equivalent of seven (7) years of full-time experience in case management/transition of care or a related field, including two (2) years experience in a supervisory, administrative, consultative, managerial or executive capacity.

NOTE:
Required degrees must have been granted by an accredited school, college or university or one recognized by Roswell Park Comprehensive Cancer Center as following acceptable educational practices.
