Compensation: $24.54/hourly
The Jail Navigator role is embedded within Compass Health and local jails, offering support in the jail setting for individuals with behavioral health needs or presenting in a behavioral health crisis. The Jail Navigator is responsible for coordination between the jail, court, behavioral health provider teams, and community resources to implement appropriate post release plans and ensure all opportunities for diversion are explored. The Jail Navigator provides coordination with appropriate team members involved to ensure connections are made and needed level of care is implemented upon release. The Jail Navigator is a part of a person-centered integration team and works with local systems to coordinate care for individuals experiencing behavioral health crises.
ESSENTIAL FUNCTIONS - JOB SPECIFIC:
* Coordinate with Community Behavioral Health Liaisons regarding Behavioral Health Courts or other specialty or diversion courts, Crisis Intervention Teams (CIT) initiative, and post release plans to ensure appropriate level of care is implemented upon release
* Collaborate with local behavioral health providers and entities in the community
* Provide consultation on behavioral health issues for jail personnel
* Assess individuals with behavioral health needs and supporting with de-escalation as appropriate
* Support or facilitate support for individuals with behavioral health needs with accessing needed resources and services
* Aid individuals with behavioral health needs with completing needed paperwork to explore diversion opportunities, community resources, and behavioral health services both during jail stay as well as upon release
* Assist jails with navigating inpatient hospitalization process when appropriate for those individuals who present a likelihood of serious harm due to mental disorder
* Support individuals with behavioral health needs with accessing behavioral health resources
* Participate in critical incident de-briefs for jail staff
* Complete all mandatory documentation, as noted by supervisor
* Act as a member of the multi-disciplinary team
* Other duties as assigned by the Behavioral Health Liaison Supervisor
* Bachelor's degree required
* Master's degree in a human service field preferred
Minimum of one year of work experience in the courts, police, and/or human service field required
QUALIFICATION(S): LICENSURE/CERTIFICATION: Licensed Master Social Worker, Provisionally Licensed Professional Counselor, LPC, or LCSW preferred
