Location: Salem, OR

Description:
Job Title: Nurse Practitioner (NP) - In-Home Wellness Risk Adjustment

Location: Marion County, OR

Shift: Monday-Friday, 8:00 AM - 5:00 PM (typical corporate schedule; flexibility available)

Salary: Highly competitive and based on experience.

Experience Requirement:

• Accepts new grads but requires relevant experience as an RN with either homecare or patient care.
• Previous in-home risk assessment experience is a plus but not required.

Responsibilities:

• Conduct annual wellness visits and health assessments for a population with chronic conditions.
• Deliver patient health education opportunities.
• Assist in closing quality care gaps (e.g., screenings and labs).
• Collaborate enthusiastically to enhance care delivery.

Qualifications:

• Valid unencumbered NP License for the state where you will be working.
• Any certification/specialty accepted.
• Patient care experience.
• Bilingual skills are a plus.
• W2 full-time employment.

Travel Expectations:

• Most of the work within the county, but occasional travel to other areas of the state for assessments (assignments given 2 weeks in advance).
• All travel expenses (including hotel and food allowance) are covered.
