Location: Albuquerque, NM
Description:
Job Title: Hospice RN Case Manager

Duration: 13-week assignment

Schedule: Day Shift, 8 hours per day (Monday to Friday)

Location: Albuquerque NM (Home Health, location to be determined)

Job Summary:

As a Hospice Registered Nurse (RN) Case Manager, you will play a vital role in providing compassionate and comprehensive care to hospice patients. Your primary focus will be to ensure patients can spend quality time with their loved ones and engage in activities that bring them joy. By building trusting relationships with patients and their families, you will empower and educate caregivers on providing exceptional care. Collaborating with the care team, your goal will be to deliver comfort, peace, and dignity to patients in their end-of-life journey.

Responsibilities:

• Provide holistic and compassionate nursing care to hospice patients
• Develop individualized care plans and regularly assess patients' physical, emotional, and spiritual needs
• Collaborate with interdisciplinary teams to coordinate and deliver comprehensive care
• Educate patients and families on symptom management, end-of-life care, and available resources
• Administer medications, monitor vital signs, and ensure patient comfort
• Maintain accurate and up-to-date documentation of patient assessments, interventions, and progress
• Offer emotional support and counseling to patients and their families
• Coordinate with community resources and services to enhance patient care and support
• Advocate for patients' rights and ensure their wishes are respected

Qualifications:

• Valid RN license in NM
• Minimum of two years of experience as a registered nurse, with hospice experience preferred
• Current driver's license for travel within the assigned territory
• Current CPR and BCLS certification
